Lesion Treatment Using Two Lasers

Moles are skin lesions that appear when melanin-producing cells abnormally increase in number in the epidermis or dermis.

While they may appear as simple hyperpigmentation on the surface, the treatment approach can vary depending on the depth and location of the lesion.

The Erbium-YAG laser is effective in precisely refining the skin surface. This allows for the removal of shallow moles close to the epidermis with minimal burden on the surrounding skin.

This procedure can lower the risk of scarring while offering aesthetic improvement, making it particularly useful for mole removal on the face. After mole removal, a CO2 laser is used to thermally refine the root part of the lesion. This process can help reduce the possibility of recurrence by treating melanin cells that may remain deep within the skin. It helps reduce the risk of 'recurrent moles,' which reappear when the surface is removed but root tissue remains.

CO2 laser finishing treatment is especially considered more carefully in situations such as:

-When the mole is slightly raised or feels thick to the touch

-When a mole has previously reappeared in the same area

-When diagnosed with a nevus (mole) located deep in the dermal layer, such as blue moles or gray-brown moles

Flat warts are skin conditions caused by human papillomavirus (HPV) infection. They typically appear as thin, wide, spread-out, small brown or flesh-colored lesions on the epidermal layer. They tend to occur when immunity is weakened and can spread to other areas through self-contact, so early management is important.

The Erbium-YAG laser helps to uniformly remove the epidermal layer. This allows for widespread flat warts to be

managed efficiently in one session. It is expected to remove them uniformly while reducing the burden on the skin in the lesion area. However, due to the viral nature of flat warts, there is a possibility of recurrence if only the surface lesions are treated. In such cases, the finishing process of applying subtle thermal stimulation around the lesion with a CO2 laser can clinically contribute positively to reducing the recurrence rate by further treating virus-infected cells that may remain in and around the epidermal layer.

CO2 laser finishing treatment may be particularly necessary in the following cases:

-When the lesion's border is not clear, or it feels widely spread and flat to the touch

-Individuals with weakened immunity (e.g., due to stress, lack of sleep, etc.)

-When there is a history of flat warts recurring multiple times

-When the lesion is located in areas prone to frequent contact or easy spread, such as hands or face

Milia are small, white, cyst-like lesions that form when keratin (a skin protein) becomes trapped beneath the skin surface. They commonly appear around the eyes, on the cheeks, and forehead, and unlike sebaceous cysts, they are not related to infection or inflammation.

The Erbium-YAG laser is considered a useful method for removing milia. It precisely ablates the skin surface to remove the thin epidermal layer surrounding the milia, creating a pathway for the trapped keratin to be naturally expelled. Afterward, lightly finishing the lesion area with a CO2 laser can help reduce the possibility of keratinocyte proliferation and prevent milia from reappearing in the same spot. However, excessive thermal stimulation on the skin can cause hyperpigmentation, so precise intensity control is crucial.

Situations where CO2 laser finishing can be particularly beneficial include:

-When milia frequently recur in the same location

-When there is a history of the lesion re-elevating during the recovery process after treatment

-When expecting more even and smoother skin during the recovery process after removing multiple milia

Syringomas are benign skin lesions that arise from an excessive proliferation of sweat gland (eccrine gland) tissue in the dermal layer. They are characterized by multiple small, bead-like lesions appearing under the eyes, on the cheeks, and forehead. They often have a genetic predisposition or are commonly observed in women in their 20s and 30s.

The Erbium-YAG laser can finely and delicately treat the epidermis. This makes it suitable for managing syringomas in sensitive areas like around the eyes. By refining the skin surface and removing the upper part of the sweat gland lesion, it can reduce the burden of scarring or hyperpigmentation after recovery, thereby enhancing aesthetic satisfaction. Subsequently, applying heat using a CO2 laser is a method that

involves a finishing process that provides subtle stimulation to the deeper parts of the lesion. This can help reduce the activity of sweat glands within the dermis, and

can have a clinically positive impact on lowering the possibility of syringomas reappearing in the same area. However, due to the nature of these lesions, they often lie deep, which may necessitate multiple treatment sessions.

In the following cases, combining with CO2 laser finishing can lead to expected improvements:

-When multiple syringomas are deeply located under the eyes or on the cheeks

-When there is a history of recurrence despite having received multiple laser treatments previously

-When considering an approach that helps reduce recurrence with a single treatment

Skin tags are benign skin lesions that form due to an excessive proliferation of fibrous tissue and blood vessels in the dermal layer. They commonly appear in areas where the skin folds, such as the neck, armpits, under the breasts, and eyelids, observed as small, skin-colored protrusions. They are painless but can become irritated by friction. Generally, CO2 lasers are effective for skin tag removal, offering cutting and hemostatic effects that allow for quick treatment of multiple lesions. However, for small and thin skin tags, an approach that involves precisely refining the lesion first with an Erbium-YAG laser, followed by CO2 laser finishing of the root part, can also be considered. This combined approach helps manage the root tissue of the skin tag, reducing the likelihood of recurrence in that area. Thermal stimulation can positively influence the regenerative environment by refining fibrous cells and microvessels within the tissue.

Situations where CO2 laser finishing should be considered together include:

-When skin tags appear in areas of frequent friction, such as the neck or armpits

-When there is a history of recurrence in the same area after skin tag removal

-When the root of the lesion is deep or wide, and it is judged that a raised mark remains even after removal

Thus, tailored to the depth and characteristics of the skin lesion,

the approach of applying two laser treatments together

is referred to as 'Dual Laser Technique'.

This method can help increase satisfaction with the treatment results.

After mole removal, how long should I keep the Duoderm (healing patch) on?

Mole removal is a process that removes the outer layer of the skin, which can leave tiny wounds on the skin.

Therefore, properly protecting and managing the treated area after the procedure is an important factor for recovery.

Keeping the treated area moist, in particular, can help speed up recovery and reduce the possibility of scar formation.

For this purpose, you might consider applying Duoderm, which is a hydrocolloid dressing.

Duoderm helps new skin growth by keeping the wound area moist, slows down scab formation, and reduces the risk of scarring.

It is recommended to apply Duoderm immediately after mole removal, change it every 3-5 days, and consistently care for the area for at least 7 days.

You can expect the total care period to be about 10 days.

If using Duoderm is difficult or if you develop a skin allergy, you should frequently apply a regenerative cream or moisturizing gel to prevent dryness. Additionally, it's important to carefully apply sunscreen with an SPF of 50 or higher and avoid touching the treated area directly.

Post-mole removal care is an important step that goes beyond simply protecting the wound; it can affect the overall outcome of the procedure.

Neglecting aftercare can lead to scarring, so it's important to continue caring for the area diligently after the procedure.

We provide detailed guidance on post-procedure care to help our patients' recovery.
