Pinhole Technique for Hypertrophic Scars: The Importance of Individualized, Customized Approach

Scar improvement is difficult to achieve with just a specific laser treatment.

The Pinhole Technique is a treatment method that carefully approaches each scar, considering its unique characteristics.

1. CO₂ or Erbium YAG Laser, selected according to the treatment area

For large and firm scars, a CO₂ laser can be used to provide relatively strong stimulation.

Conversely, for thin and sensitive skin areas, it may be more appropriate to use an Erbium YAG laser for a more delicate approach.

The medical team performing the procedure carefully examines the characteristics of each area and the scar's condition to determine the appropriate laser.

2. The spacing and size of the pinholes are also individually adjusted.

Generally, pinholes are treated at intervals of 1 to 2mm.

However, depending on the shape and condition of the scar, the spacing may be widened to more than 3mm, or sometimes treated more densely.

The medical team directly checks the location, shape, and thickness of the scar to adjust the spacing and depth of the pinholes.

3. The laser ablation depth is also precisely planned, considering both the upper and lower dermis.

A decision is needed on whether to apply the laser shallowly or to provide deeper stimulation.

For example, whether to stimulate only the upper layer of the dermis,

or to proceed with treatment deeper into the collagen layer of the lower dermis.

This treatment range is individually determined based on the medical judgment of the treating physician.

This delicate process is only possible with skilled know-how and clinical experience.

4. Not a mechanical procedure; the medical team personally handles everything from planning to treatment.

The medical team consults directly with the patient to assess their skin condition and then plans the treatment.

They meticulously examine the scar lesion,

carefully design the position and spacing of the pinholes to match the characteristics of each scar,

and delicately adjust the laser intensity to the patient's individual skin condition.

The recovery process after the procedure is also continuously monitored.

Furthermore, detailed guidance is provided on whether additional treatments are needed and appropriate treatment intervals.

Principle of Pinhole Technique for Hypertrophic Scars

The Pinhole Technique

uses a CO₂ or Erbium YAG laser to create microscopic channels at 1-3mm intervals in raised scar tissue. This helps separate excessively proliferated collagen fibers,

loosening the firm structure of the scar and encouraging it to naturally flatten.

This procedure can penetrate deeper into the dermis than typical fractional lasers. Rather than merely improving the scar surface, it can be more suitable when applied to papular or hypertrophic scars located deep within the dermal layer.

The procedure primarily utilizes CO2 and Er:YAG lasers. Our medical team carefully examines the characteristics of the scar area and customizes the treatment to suit the patient's needs.

Are there any precautions after the procedure?

QnA

Small scabs may form on the treated area, but it is best not to forcibly remove them and allow them to fall off naturally. Please avoid using scrubs or exfoliants that could irritate the skin. When going outdoors, you must use sunscreen.
